Evaluate the health care environment for its therapeutic value, based upon Maslow’s Hierarchy of Needs and Hildegard Peplau’s theory of Therapeutic Use of Self. To critique the environment, use each of the following criteria along with your observations and reference materials to write a scholarly paper. Basic physiological needs are fulfilled. Until these needs are fulfilled, a person cannot progress to the higher levels of functioning. Who assists the clients in fulfilling these needs? Are these times used as opportunities for therapeutic interactions? What technique do you observe that serve to promote optimal health and well-being for clients? Secondary but essential needs at the second, third, and fourth levels of Maslow are addressed. What are the needs at these levels? How are they met in this milieu? Do you observe that any clients exhibit the highest level of Maslow’s Hierarchy? Here make sure you discuss safety and security, belonging and attachment and self-esteem and self-efficacy, the highest level being Self-Actualization and Transcendence. The physical facilities are conducive to achievement of the goals of therapy. Does each person have sufficient privacy and space? What are the private spaces like-are there personal items in the rooms, or is it a more clinical environment? How are the furnishings in the communal areas arranged? What type of lighting is there in both the private and communal areas? Does the arrangement promote or hinder interpersonal interaction and communication? Using your senses of smell, hearing, and vision, imagine how you would feel in this environment? What is the ratio of staff to clients? How is reality orientation promoted? (i.e. large clocks, calendars, posted daily schedules). Interpersonal interactions with the staff are continually therapeutic. What ways do you observe the nurses and other staff use to increase self-esteem in the clients? What are the appropriate ways in which they deal with the client’s anger? How do the nurses recognize signs of increasing anxiety and intervene to stop progression? What is the interpersonal structure of the environment? Is it informal, family-like, or is it a more rigid, clinical atmosphere? A structured program of social and physical activities is scheduled as part of the treatment program. What activities are scheduled that you see as therapeutic? How are clients encouraged to participate? Are there various group activities selected for clients with specific needs (i.e. exercise program for a client who expresses anger inappropriately, or a music group for those with dementia)? Clients’ rights to choose and participate in a variety of treatments are preserved. How is this done? What examples did you observe? In addition to the above reflections answer the following questions: What are the facility’s greatest strengths? What is the facility’s greatest weakness?
